witnesses, other than your proxy, who have no interest in your estate,
must sign the completed form.

Give a copy of the
completed form to your physician, to be placed in your medical records.
Ask that a flag be placed on your form, so it can be easily found.

Give a copy of the
completed form to your proxy, a family member and/or a friend.

Carry a wallet card noting
that you have a Medical Directive form and where it can be found.
|
|
Keep the original copy of your
completed Medical Directive that can be accessed if you suddenly become ill. |
